UPDATE: Tessa Rivera has been found and is in the process of being reunited with her mother, authorities said Tuesday.

The Chelsea Police Department is urging residents to be on the lookout for 16-year-old Tessa Rivera of New Bedford, who went missing on July 17.

She was last seen wearing a gray hoodie, black sweatpants, and black Crocs, the department said.

Police say Rivera ran away from her mother while at Massachusetts General Hospital Chelsea. A press release published Monday says she has a history of suicidal statements. Rivera does not have her phone and is unfamiliar with the Chelsea area, according to police.
